Please have your child's medical provider fill out this form and submit any supporting documents. Please also request a copy of your child's immunization records.  

Use this document to inform the provider of your child's typical feeding schedule, habits, likes, dislikes and allergies.  

Please submit this document to give consent or refuse to allow your child to appear in photographs or videos on Passport to Learning website and social media platforms.

Only complete if applicable. This document is to be completed by the child's physician or specialist if there is a need for specialized care accommodations based on an area of concern or medical need. 

Please submit if your child requires any daily medicines to be administered during hours of care or to give consent for your child to receive over the counter medicine if needed and with parental approval.
